Release of Information 2 Within healthcare we need to address the steps that are required to release patient information. There is a request for patient information release, and to fulfill this request, there must be a verification process ensuring this is the correct patient. Validating disclosure involves identifying a patient's full name, date of birth and or medical identification number. Monitoring and tracking this process is necessary to ensure quality care. Ensuring to protect this sensitive information of the client based on HIPAA guidelines. After a thorough check, the personal health information was released. Lastly, Generating an invoice of patient information release upon request completion. Determining whether authorization is required is based on who is requesting this patient information. If a physician is assisting a patient and is a covered entity to use and disclose PHI, then they can access this information without the individual's authorization for the use of treatment, payment, or health care operations. There is an authorization form to help secure and protect the patients' healthcare information. Patients have a right to revoke the authorization at any time. Release of information (ROI) allows protection to the consumer and provider in releasing PHI in relation to HIPAA compliance. Effective communication with the patient and provider will ensure privacy, prevent discrimination, enhance autonomy and quality of care. Patient’s rights are to be free from harm, which includes unnecessary physical restraint, abuse, or neglect. Medication should not be used as punishment to interfere with a treatment program. A right to treatment services which promote the potential of the person to function independently. Treatment should be provided in ways that are least restrictive of the personal liberty of the individual” (CDSH, 2024). By law, it is the patient's right to be informed of their health status, involved in the planning and treatment of care, and able to request or refuse treatment as they see fit. There are criminal violations that are handled by the federal department of justice of improper ROI that could range from 50,000 to $250,000 as well as prison time up to
