Community Assessment – York County, Pennsylvania
A major influence on health in a community is the availability of services and opportunities within the neighborhood. Public health can be further influenced by the social environments. Accessibility to health care along with education and employment opportunities are fundamental factors to be considered (Riegelman & Kirkwood, 2015). These elements create both advantages and disadvantages for those living within the neighborhood. If we want to improve public health outcomes, we must consider and evaluate these along with many other contributing factors.
Setting Description York County, Pennsylvania is an urban community which, according to the US Census, covers 904.18 square miles with an average of 481.1 persons per square mile (State and County QuickFacts). The county is made up of approximately 73 cities, townships, and boroughs ranging in size. York City is the largest population of 43935 while Yorkana borough is smallest with a population of only 229. There are two hospitals as well as several urgent care and outpatients healthcare facilities.
Demographics
The US Census estimates York County population at 440,755 (State and County QuickFacts). The bulk of the population, 56 percent, falls within the age range of 18-64. This closely corresponds with the state of Pennsylvania as well as the US statistics for the age of population. Additionally, only 15.9 percent of the county is over the age of 65. Furthermore,

Our health is also determined by the health and social services we receive, and our ability to attain high education levels, food and safe housing, among other factors.
Butler County is a predominately Caucasian county, located north of Pittsburgh, in Western Pennsylvania. The culture of Butler County is shaped by the limited diversity of the area. Much of the population residing in Butler County are also somewhere between the ages of 18 and 65 (Census Bureau 2015).
Pennsylvania was one of the original 13 colonies, it became the second state administrated into the union on Dec 12 1787.Like other states Pennsylvania has a motto,Pennsylvania’s motto is “Virtue,Liberty, and Independance”.Pennsylvania’s total population is about 12,703,379 (but it’s ever changing) there is about 49,188 in Pennsylvania’s capital Harrisburg which is about 1/240 of the total poppulation. Pennsylvania’s state bird is the ruffed grouse, the state flower is the mountain laurel, and the state tree is the eastern hemlock. Pennsylvania Has many nicknames such as the keystone,oil,coal, and quaker state and state of independance. The origin of Pennsylvania’s name is from it’s founder Quaker Penn as Pennsylvania in dutch means “Penn’s Woods”.
“Failure to acknowledge, and more importantly, to understand the role of Social determinants of Health (SDH) in health and access to health and social services will hamper any effort to improve the health of the population.” (Ompad, Galea, Caiaffa, & Vlahov, 2007). Unemployment, unsafe work environments, globalization and the inability to access health systems are some social determinants of health. Social factors would be considered place of residence, race and ethnicity, gender, and socioeconomic status are also part of social determinants of health. In 1702 Philadelphia was established by Great Britain, however modern-day Pennsylvania was granted statehood 85 years later in 1787. In between this 85 year period a lot happened including two important Pennsylvania wars. Everything basically stays the same until 1714 when something happens and Britain controls most of modern-day Pennsylvania and a group of indians called the Iroquois are driven out of modern-day Pennsylvania. Some may ask what happened and the answer I would tell them is that they just expanded and explored nothing much. 40 years later in 1754 the first war that is important to modern-day Pennsylvania, The French and Indian war. Health disparities are not based on income but on location. In Chicago, health disparities are a major problem because of segregation. While, on the Northside, someone is less likely to experience health disparities, it’s different on the South and West Sides. When someone looks at a map, they do not see how segregated a city is, but when someone is analyzing a certain neighborhood in Chicago, they will be able to see how segregated the city is. Racial and ethnic health disparities plays a significant role when it comes to those living in a what is considered a low- economic community because people are more susceptible to poor air quality, high blood pressure related to stress, and violence. Inaccessibility to healthy foods forces people to have to eat unhealthy and struggle with the risk that can lead to obesity or high cholesterol down the road (Noonan, A et al ,2016). Also social racism with having limited income, and education can cause people to act out in a negative way due to being frustrated and can cause one to act out in a violent way which can cause a spike in crime. Education level, employment, income, family and social support, and community safety are all part of the social and environmental determinants in healthcare (Senterfitt JW et al., 2013). Public health has concentrated on improving clean water supplies, healthier housing, sanitation, workplace safety, safe food, and access to medical care to increase life expectancy (Senterfitt JW et al., 2013). Social and economic factors are not only the largest single predictor or driver of health outcomes, but also strongly influence health behaviors, the second greatest contributor to health and longevity(Senterfitt JW et al., 2013). “Building family, building business, building future[O1] .” This is the New Salem town motto. Three senior nursing students assessed the rural community and have determined the motto is fitting. New Salem is only 20 miles from the larger, urban city of Bismarck, but that does not mean that New Salem has nothing to offer. New Salem has a grocery store, a school, several churches, and recreational activities available. New Salem also has several different attractions including a statue of world’s largest Holstein cow also known as “Salem Sue.” Diving deeper into New Salem, students explored the civic core, subsystems, perceptions, and priority needs of the community.

With that said, it is thought that the drug trafficking that goes on in Dane County are from these two nearby cities. Another concerning factor would be that of Health. Although Dane County residents are quite healthy overall, by digging deeper into the data we find significant differences in the rates of death and disease (Health disparities) amongst her residents. The rate among our black residents of the community is more than double, which is concerning. Nonetheless, I believe with special attention to the vulnerable population in the community health disparities. Income, education, racism, environment, access to adequate and affordable housing, likewise, safe neighborhoods are all necessary in order to design an individual chances for a long and healthy life. It is my hope, that collaboration and partnerships between community members and government agencies, creating opportunities to improve health through programming and policy change will influence the community and the environment in which we live, work, and
Two counties in Pennsylvania are: Lancaster and Philadelphia. Lancaster County consists of small towns. This county is rural with many areas of farm land. Also, this county offers many attractions. The town Adams is the antique capital.
The data has shown that there is an ongoing problem with obesity in DeSoto County and many counties in Florida. The State of Obesity, reports that Florida 's adult obesity rate is 26.4 percent, up from 20.7 percent in 2004 and from 11.4 percent in 1990("Florida", 2014). In 2011 for DeSoto County, the percentage of obese females was in the worst 25% of all counties at 44%, while the percentage of obese males was in the middle 50% of all counties at 38.2%. The national average in 2011 was 36.1% for females and 33.8% for males. Excess weight is considered to be one of the major causes and determinates of future health problems. These problems include diabetes, hypertension, and heart disease among others. These issues cut across race, gender and age. If you are a black person aged 45-64 you are more likely to either have one of the diseases attributed to obesity or have the highest risk of developing one of these diseases. There is some good news among the data when it comes to children, the CDC reports that between 2008 and 2011 Florida 's rate fell from 14.1% to 13.1%, a statistically significant decrease according to the CDC analysis. The social determinants of health topic area within healthy people 2020 are designed to identify ways to create social and physical environments that promote good health for all. Healthy attitudes start from homes, schools, workplaces, neighborhoods, and communities. People practice good behavior by eating well, exercising, getting scheduled immunizations to prevent infectious diseases, drinking clean water, breathing air free of pollution, attending checkups with medical doctors and going for disease screening (Healthy People, 2014). The quality of our schooling, the safety of workplaces, our social interactions and relationships, explain in part why Americans are healthier than others. The determinants of health fall under several categories
