Journey’s intake unit completes all initial assessment to assess client for different level of care and accurately assign them to the most appropriate unit. The different level of care ranged from intensive inpatient care unit to medication only. As a practicum student, I worked in the outpatient services unit. The assessment was completed via an online program with an intake staff and the client, usually a week or two before the first scheduled appointment with a clinician. The patient that I have chosen for my diagnostic reasoning paper is a 47-year-ol-Hispanic female. The presenting problem that I have chosen to use as my patient’s chief complaint is back pain. The only other clues that I have to use in order to help narrow my focus is that she is a female, she is 47-years-old, and she is Hispanic. I do not know how long she has been experiencing pain or how severe her pain is. Given these parameters, I will “cast a wide net” as I evaluate my patient and create my list of differential diagnoses.

The psychiatric clinical rotation at Clinch Mountain House was a very prodigious experience. This facility is unique because of the role it plays in the community; it serves to provide daily recreation for the members of the community who suffer from mental illnesses. This facility focuses on providing essential social skills to this community and accomplishes this goal by providing social interaction, a structured schedule, and daily assignments for each member.
Differential diagnoses are developed by a clinician upon learning of the chief complaint. One must begin to develop the possibility of potential diagnoses mentally to guide the care provided to the patient. These potential diagnoses are developed by the care provider and are often based on one’s past clinical experiences, awareness of the illness and a clear understanding of the patient’s complaint (Goolsby & Grubbs, 2014). The care provider with experience may develop these diagnoses independently and others with less experience may utilize evidence-based resources and clinical guidelines to aid in this process (Goolsby & Grubbs, 2014). Although, Susan changed her major several times she never finished college. Susan chose different majors to try and find what fit best for her. With all of the majors, she tried she couldn’t find one that she enjoyed long enough to complete the program. Susan went to community college for three years. During the three years she was enrolled she majored in nursing, business administration, education, and sociology. Susan admits that she has never been really good at school, and she only chose two of the majors because her friends chose them.

Patient is a nineteen-year-old college student working as a CNA. Patient’s has multiple chronic conditions such as Celiac Disease, Spina Bifida, Sjogren, Malabsorptive Disorder, Gluten Intolerant, Anemia, MTHFR Gene Mutation, deafness on right side, and Protein S deficient. Patient also has PTSD which contributed to depression, anxiety, and binge eating. Patient also complained of tiredness and being lethargic for 4-5 years and was put on Zoloft for depression. Patient is allergic to nickel and aloe vera. Patient is also at risk for breast cancer due to family history. Patient’s medications are Sertraline, Oxybutynin, Magnesium, Vitamin D, Vitamin E, Iron, and Vitamin B.
The fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) divides the substance-related disorders (SRDs) into two sub-categories, namely, substance-induced disorders (SIDs) and substance use disorders (SUDs) (McCarron, Xiong, & Bourgeois, 2009). SIDs relates to the medical conditions that are caused by the use of substances. These conditions include intoxication and withdrawal syndromes. SUDs are defined as a patient’s recurrent behavior to use alcohol and/or drugs that cause clinical and functional impairment, for example, malfunction at school, work, home, and health problems (McCarron, Xiong, & Bourgeois, 2009).
The importance of creating a learning environment that fosters learning, interaction, engagement, and motivation. I found that classroom management, rules, routines, and community building activities very fitting of this standard. The Classroom Management Plan, Classroom Expectations, Classroom Climate Activity, and a Student-Centered Discipline Plan will help the environment in the classroom.
