My objective is to integrate the theory of nursing I have learned in class with the practical portion of clinical in an acute hospital setting.

I was able to achieve this objective this week by performing total care with the help of my instructor, nurse and PCA on a patient that was admitted for mental status change and diagnose with UTI and Pneumonia. I was also able to practice charting in the hospital’s powerchart system and passed medication under the guidance my instruct

There has been growth in my performance and effectiveness of my communication and organizational skill at clinical from clinical day 1. I intend to improve in this area with continuous practice and following the organization plan my clinical instructor provided.

Mr. Harris expressed to me the key factors in being successful in his workplace. He stated that the employees should think about the patients first and make the quality of their care the first priority. To be unsuccessful in his clinic, he states that employees would not take pride in their job and not care about the quality of their work. He expressed to me that a manager should most importantly succeed in having open communication with all employees and stakeholders. Keeping an open line of communication with employees makes for a positive workplace environment. Harris explained that this can be done by checking in with them often to make sure the employees aren 't having any issues and are satisfied with the staff’s work. He also interprets that you should let them know that you are always available for them if issues or questions arise.
My second clinical day at Mercy Defiance Hospital, progressive care unit, was overwhelming but rewarding and very educational. Through the 8 hours I was there, I learned a lot. I engaged myself in many self-directing learning practices to futher promote my professional growth. My first time interaccting with my patient,I was joined with my clinical instructor. While in the room, I got to listen to to my patients heart, lungs, and bowel sounds. I was able to see and examine the patients ostomy bag, as well as help them to the bathroom. There were ways that I can improve my professional growth, suchas,having the confidence to not second guess my self about things to do in assessments. For example, I know how to assess the lungs, I should not second guess where to listen for the sounds. One of my goals for next week dealing with professional growth would be to have the confidence to know what I have learned and use it. My second one being to go into the room with a confident attitude and believe in myself.

I successfully manage all timekeeping for my staff, as well as ensuring they have a clear understanding of the Institution and Access Operations expectations for time and attendance. I have created a new patient access checklist to assist with orientation of new team members within the center. To enhance our patients’ experience monthly call monitoring is performed and feedback is given to staff. In identifying customer service areas needed for improvement, staff has been successful with improving contacting patients within four hours, reduce the call abandonment rate, and increase the number of patients given same day/next day appointments. My plan for the upcoming year is to become a better leader by focusing on being better organized and prepared, improve on meeting deadlines and communication. Formative evaluations have been very beneficial for me in pre-clinic. During the start of pre-clinic, I did not realize how unsure I would be when it came to handling instruments when working in my classmates’ mouths. The formative evaluations help me to be a little more at ease as the instructors guide me into the right direction. The formative evaluations have enabled me to improve in all areas we have learned thus far in pre-clinic.

Critical thinking and clinical judgment are important skills that professional nurses use in every day clinical setting. In 2012, a mix method qualitative study by Dr. Jeanne Mann was done to evaluate the effectiveness of educational strategy to develop clinical judgment skills in nursing students. Prior to my clinical day, February 21st, 2017, I created clinical objectives to complete to enhance my learning. The clinical objectives I created for myself were examine lab values earlier in the shift, explore deeper into the patient’s history and pathophysiology of my patient’s conditions, and manage my time better. I kept these objectives in mind throughout the day to be productive.
Critical thinking and clinical reasoning are essential skills to develop a sound clinical judgement. Nurses use critical thinking models and processes to support and organize the interventions that they perform in the clinical setting. In the scenario of Mr. Patient requesting for a “Do Not Resuscitate” order, I used the nursing process to serve as the framework of the patient care and the Lasater’s Clinical Judgment Rubric by Kathie Lasater (2007) to guide my reflection. In this paper, I will describe the skills that were used in relation to the nursing process, provide analysis of the model and application of learning in terms of CNO competencies, and evaluate my clinical decision-making in terms of patient outcomes. While, the nursing process, served as a guide to organize my thought process in relation to the overall picture of the patient, I also incorporated thinking structures such as ABC and Maslow’s Hierarchy Of Needs to determine the prioritization in each activity. Furthermore, I utilized the Structured Prebriefing Worksheet by Karin Page-Cutrara (2015) to make inferences from the data before engaging in the scenario.
