There will be many scenarios that can challenge a counselor. However, when there are specific cases that involve harm to the clients’ person or another a counselor must follow the processes outlined in the bylaws of the American School Counselor Association. These outlined processes serve two important purposes, the purpose of protecting the client and the purpose of protecting the counselor. This paper will answer questions in response to the scenario of Angela. Scenario: Angela is a 16-year-old sophomore referred by a teacher who states that she is not functioning well in class and appears to be depressed. During the initial session with Angela, there was an assessment of Angela’s suicidal thoughts. Angela admits that she has had suicidal feelings off and on for the last few months since she broke up with her boyfriend. She said that they had sex and that she is now feeling remorse and guilt, because it is against her family values. She also believes it is her fault that they broke up. What ethical considerations and processes must you consider in working with Angela? There is an ethical duty to report a client of any age when there are reports of suicidal attempts or ideation. Confidentiality is a consideration, but the safety of Angela is the first priority. There is a legal and ethical duty to report if there is a foreseeable harm (Remley, T. P., & Herlihy, B., 2010). Working at the Elmcrest Children’s Center this semester has opened my eyes to so many different issues that children face in our own community. The children that are sent to Elmcrest go through psycho-social evaluations before they are fully admitted to see exactly how the child reacts to certain situations and stimuli. During my time at Elmcrest, there have been kids that have been discharged to other programs due to the fact that they had dealt with suicidal thoughts and needed more intensive care than Elmcrest was able to provide. L is a 16 year old, Caucasian female student admitted to the adolescent mental health unit of a major Brisbane hospital due to active suicidal ideations manifested by taking a recent overdose. This was also accompanied by thoughts of harming herself and ending her life. L has a history of suicidal ideation, an eating disorder and self-harming. L was picked up from school by her stepfather and later disclosed that she didn’t want to be here anymore and wanted to die.
The ethical and legal ramification of this case is the medical employee forced the patient to take medication that she refused to take. The employee poured medication down the patient throat by holding her chin. In fairness, this is resident abuse, so I would suspend the medical employee license indefinitely because the employee is aware at this against the law. It will be okay the medical employee to get a court order for a patient that is endanger to themselves to give the patient medication will they disagree. The message that I will send is this type of action is not tolerated, and anyone will have their license suspend, and subject to prison time.
On March 22nd, I completed an evaluation for a 16-year-old Caucasian female, who was referred to the Crisis unit by Hampton Academy School. Today was the patients second day attending this academy, and she reported suicidal ideations—without any plan— to the school social worker. The patient reported that she has been "feeling suicidal, not okay, and at her breaking point "(Anonymous, personal communication, March 22, 2016). The Patient stated that her Grandfather passed away—in the home—in February, and she constantly relives the experience of seeing him lifeless in their home, as well as her past bullying experiences. The patient reported that she can still visualize her grandfather, as well as hear the perpetrator calling her a "fat bitch"(Anonymous, personal communication, March 22, 2016). The patient stated that she "doesn 't really have a plan to commit suicide, but there is one in the back of [her] head: pills, bleed out, or cut really deep"(Anonymous, personal communication, March 22, 2016). Patient reported that she does cut, burn, or scratch herself sometimes to relieve the psychological pain, but confirmed that those are only her coping mechanisms. Patient reported that she doesn 't always feel suicidal, but she feels this way on and off. The patient denies any past suicide attempts, homicidal ideations, and hallucinations. In May 2016, the Foundation for Accountability and Civic Trust (FACT) filed a “complaint and request for an investigation with the Federal Election Commission (FEC or Commission) against Patrick Murphy and Floridians for a Strong Middle Class (Floridians), an independent expenditure only committee registered with the FEC and formed [as super PAC] to support Patrick Murphy’s campaign for U.S. Senate …” (Whitaker, 2016a, para. 1). The complaint holds that Murphy and his father, Thomas P. Murphy, Jr., contributed more than half of all the total contributions to the super PAC. According to Whitaker (2016a), Mr. Murphy, part owner of their family business, Coastal Construction Group, reportedly contributed $300,000 to the super PAC. Murphy’s father, CEO of Coastal Construction Group, reportedly contributed $200,000 to the super PAC (Whitaker, 2016a).
Ms. Hill is a 13 year old female who presented to the ED following a suicide attempt. Ms. Hill attempted to overdose on 12 12.5mg of Phenergan and 2 500mg Amoxicillin with the intent of ending her life. Dr. Snyder requested an assessment. At the time of the assessment Ms. Hill was resting, however was awaken by her mother and father who were both in the room at the time. Ms. Hill denies current suicidal ideation, homicidal ideation, and symptoms of psychosis. She informs this clinician she has been dealing with depression for a few years due to bullying. She expressed recent feelings of sadness, tearfulness, isolation, anhedonia, hopelessness, worthlessness, insomnia, and fatigue. Ms. Hill reports last year almost being rape during the summer,
Ms. Sanders is a 13 year old female who presented to the ED With her mother for recent suicidal ideation without a plan and depression. Dr. Keith requested an assessment. At the time of the assessment Ms. Sanders denies suicidal ideation, homicidal ideation, and symptoms of psychosis. She reports feelings overwhelmed and depressed to the the many extracurricular activities she is engaged in at school and in the community. Ms. Sander also reports recently coping with becoming aware of her boyfriend of 3 months going into a detox facility this morning. Mr. Gray is a 13-year-old male who presented to the ED with after an argument with his father, which resulted in him allegedly saying he wanted to die to his sister. Per documentation Mr. Gray denies any mental health history but admits to hearing voices in the past. At the time of assessment, Mr. Gray denies feeling suicidal. He states, "I never mention wanting to kill myself to anyone, I just got angry." Mr. Gray reports family conflict as the primary stressors contributing to his current distress. Raelynn is a 7-year-old Caucasian girl who lives with her mother, stepfather and younger sister. Raelynn’s parents divorced when she was 3 years old. Raelynn was brought to the clinic because her mother found a journal depicting suicidal ideation. Raelynn wrote things like: “I want to die, I think about it all the time. I think about running into traffic, as fast as I can”. Her mother also reported that Raelynn had been talking about wanting to die and killing herself daily. When completing this crisis situation, it would be important to act quickly. I thought I would run to ask the school principal what they would want counselors to do in the situation. Then I realized that I would not have time for talking with the principal. Then, I thought about how I would react in this situation. These rumors being that Rob had acquired some outside knowledge, which in turn led to a breakthrough within the Research and Development department. I have been asked to review all relevant information, and make a determination of if MegaLab acted unethically in violation of law, and submit a draft for consideration to Human Resources an Ethics policy that can be applied to the Research and Development department.
