Hourly Rounds Reduces Patients’ Frequent Call Lights and Improves Safety. Christian Oyibe NURS 8103 Evidence Based Practice. Governors State University Professor Somi Nagaraj, MSN, DNP. June 5, 2013. The nurses were required to attend to the personal needs of the patients by asking if they want to use the bathroom or bedpan and any other needs that the patients want to be attended to. In addition to the above, the nurses check position and ask patients if there is anything he or she could do to make his or her positioning comfortable. These include identification of risks for skin breakdown and attending to such risks by providing hygiene and skin care. Finally, they ensure that patients’ call lights, side table, water pitcher and other basic needs are within the reach of the patients. In addition to that, they ask the patients if there is any other thing that they want the nurse to do, and the nurse told the patients that he or she will be back in about an hour to check on the patients. On the other hand the control group
As a Registered Nurse is a crucial component to ensure that there is effective communication between the nurse and patient. As well as being respectful to the person’s dignity, culture, values, beliefs and rights. This is because everyone is different, and due to this it is important that I am being cautious to each person’s individual needs.
Traditionally, nursing shift-to-shift reports were organized methods of communication between only the oncoming and leaving nurse, designated to a location such as the central nursing station or nook of a hallway. Shift reports can be considered the foundation of how the day is going to plan out because it introduces the patient, diagnoses, complications, medications, consults, upcoming test and the entire plan of care. These reports are full of complicated and vital information and while set in certain locations that are vulnerable to interruptions, such as the nursing station, medical errors and miscommunication are more likely to be made. The Joint Commission’s 2009 and 2010 National Patient Safety Goals (Joint Commission, 2015) included two patient safety standards, first to encourage patients to be involved in their health care plan and second, to implement a standardized communication process for handoff reports between providers. Soon after in 2013, The Agency for Healthcare Research and Quality under the United States Department of Health and Human Services introduced a set of strategies to improve patient engagement along with safety and quality in patient care. Within these strategies the new method of nurse bedside shift report was developed, which suggests nurses to conduct shift-to-shift reports at bedside in the room of each patient, rather than out of the room. The benefits of this new method were
This is a critique study on “effects of nursing rounds on patients’ call light use, satisfaction, and safety by Christine Meade, PHD, Amy Bursell, PHD and Lyn Ketelsen MBA,RN(2006)”outline as quasi-experimental research design. The author states in this article about patient’s frequent usage of call light in the hospital. Frequent call light usage seems to be an ongoing problem in the hospital setting. This study was to determine how to reduce the call light use and burn out staff, also increase patient satisfaction and safety.
When a patient becomes hospitalized, it is important to ensure patient safety and satisfaction. A process known as hourly rounding has been implemented in many hospitals as a way to achieve this increase in patient satisfaction. The hourly rounding process consists of four concepts that are covered with each hourly round. When these concepts are implemented, the nurse will anticipate the needs of the patient and prevent unnecessary and untimely call lights. Hospital implementation of this process may receive some resistance but with the proper training and education this process can increase patient safety and patient
Devote effort to focus on patient’s need for help with toileting, pain level, and positioning.
A C.N.A. can work effectively as a member of the health care team by helping a patient or resident meet their basic human needs, for example, assisting with meals. Doing so will help the patient or resident meet other needs such as a small goal with more comfort and ease. In addition, a C.N.A. can work effectively by observing and reporting any changes in behavior or health conditions when providing for the patient's or resident's basic human needs.
The bedside shift report occurs between the outgoing and oncoming nurse next to the patient’s bedside at shift change. It is intended to engage patients in the conversation and exchange of information. This gives both nurses and the patient the opportunity to ask questions and verify important information about the patient’s history and care plan.
Essential care needs present specific requirements for an individual’s survival. The Nursing and Midwifery Council (2010), suggest nurses should be able to consider patient’s essential care needs and aid them with these needs when patients are not able to help themselves. Allowing nurses to make the patient feel comfortable, during periods when they may be undergoing distress or discomfort. Nurses have to deliver essential care such as making sure patients are eating sufficient amounts of food, keeping hydrated and assisting with hygiene needs.
For the patients we can have pamphlets available at bedside to have tips on when to call a nurse for help to get out of bed and other things to look at, for instance there are many wires and tubes in a patient room and they need to be cautious of their surrounding so they don’t mistakenly fall. Patients need to know the importance of safety.
Traditionally, nurses debrief other nurses during a shift change in the hospital setting at either the nurses’ station or in a conference room. They may or may not communicate with doctors and other hospital staff using these same techniques. Some hospitals have switched to discussing the patient for shift changes purposes at the patient’s bedside, as part of a refocused patient-centered care model developed to reduce errors and to keep patients better informed in the process. The benefits of utilizing a bedside nurse patient handoff reporting process within the hospital setting greatly outweigh any possible risk or inconvenience when compared with alternative or traditional options.
The nurse generalist assumes many roles, and often the nurse fulfils these roles simultaneously each shift. Some roles of the nurse generalist include, but are not limited to: patient advocate, educator, communicator, delegator of care, leader, care provider, evaluator, and professional. Each role of the nurse generalist is essential in optimizing patient safety and quality of care. Patients and their families rely significantly on nursing staff, more so than any other discipline, to meet their physical, emotional, and spiritual needs. Additionally, the patient and their family depend on the nurse to competently and confidently manage their care, by advocating for patients’ rights and wishes to be respected, educating the patient and family

According to Goodrich and Cornwell (2008), healthcare staff needs to be able to care for patients with kindness and dignity, and also to give them the same standard of care that they would want for themselves or their loved ones. This assessment will begin by highlighting a patient from the practice; discussing the philosophies, models and frameworks, and implementing an appropriate care plan which is required in nursing intervention ( Selvin and McKenna, 2008). This will be followed by the models of activities of daily living; comprehensive assessments to indicate how the patient’s individual care needs were addressed during their hospitalisation. In order to analyse the nursing interventions, and how they can improve the patients future
To begin with, patients may be unmotivated and lose interest in their own personal needs and be unconcerned about personal hygiene. The nurse can come up with a routine for hygiene, grooming and dressing so the patient can simply follow the routine instead of having to make decisions. Furthermore, the nurse can provide patients with their own clothing if possible, to allow them to feel safe and comfortable. If the patient is kempt, it helps to improve their mood as well.
In the profession of nursing, it is always necessary to pay attention to any areas of care that might cause a potential issue. When assessing a patient, it is important to pay attention to anything out of the ordinary and take responsibility for monitoring the patient. It is important to have a good understanding of any issue that a patient might be experiencing, as well anticipating how they might react to a certain treatment or medication.
