It is no secret that communication is key when providing direct patient care in a skilled nursing facility. However, there is a noticeable lapse in the communication between the care team when providing care to the individual or groups of individuals. Two main parts of any care team are the registered nurse and the certified nursing assistant, as these are the two people whom have the most direct and impactful roles with residents in a skilled facility. The Registered Nurse and the Certified Nursing Assistant play similar roles in providing patient care, but have different roles in its entirety. I have numerous experiences volunteering at the hospital, interning for an RN and a Vice President for Human Resources, which have all led to my interest of this particular program. The seamless communication we exhibit among the various units across different disciplines helped me choose this nursing care model. No one provider can do all of this alone.” This is absolutely true that no one healthcare provider can solely take on all aspects of the patient’s care alone. Over time, studies and research have shown that patient’s outcomes and quality of care improve when healthcare providers join together as equals, it also allows for providers to bring their competence and experiences on how we can improve (Naylor, 2011).

A lack in patient-centered care arises from time constraints and poor communication. According to Sullivan (2013), patient-centered care is a representation of nursing responsibility conveyance in which the job of the registered nurse is expanded to collaborate a group of multidisciplinary unit-based caregivers. Communication might become unclear or lost if there are multiple physicians taking care of a patient. Shared decision making is a main aspect of patient-centered care (McCance, 2011). Patient-centered care respects the preferences and needs of each patient. It also allows the patient a chance to ask questions. All of a registered nurse’s primary duties are focused on the direct quality assurance of his or her patients’ care. Nurses provide the initial assessment of the patients’ illness or injury by conducting several different diagnostic tests based on the patient’s information and presenting problems. These diagnostic tests may include a temperature reading, blood pressure check, respiratory count, pulse, vision examination, reflex examinations, and an examination of the medical history. Based on information obtained from the initial assessment, the registered nurse and the physician can determine if the patient requires further examination by another physician or other medical specialist.
