(a) Briefly describe your organisation in relation to purpose, location, size and target population (250 words). I work at the age care and disability service of the ACT in Canberra. Firstly, in the ACT age care service, the setting of its organisation mission is mainly focus on the quality of service deliver to elderly people. These services are ranging from quality caring, healthy eating, quality beddings, quality delivering of medications and healthy environment with quality cooling and warm systems that have been installed in the organisation facility. Its location is quite good for the residents and employees to come to work on time, and size is worthy to continuing in receiving funds from the government. It population is reasonable to be manage by both private and government. ACT Disability service, is a multifaceted area of social strategy, both because of its size, diversity of the target population and the variety of individuals, organisations and governments involved in service provision. Frequent major strategy changes over the last two years have added to the difficulty of determining who is responsible for what, and with what consequences this could bring. ACT Disability services, operate in home care, and facility care all these facilities have installed cooling and warming environment most appropriate for those having disability. Its purpose is to ensure that disability is not inability for those individuals these services are provide. Thus what the

Following secretary of state, John Reid’s statement in 2005 declaring that social care “should be about helping people maintain their independence, leaving them with control over their lives, and giving them real choice over their lives, including the services they use. Services must recognise the changing world, our changing attitudes and our ageing population”. This assignment will analyse the present affairs in the social care world and see if the UK has stepped closer to John Reid’s vision.
Opportunities which can be addressed though senior care includes the increasing of staff including geriatric nurses. This can be done by local long-term care facilities, partnering with community colleges in training nursing staff and staff having the ability to gain the necessary accreditation needed. Long-term care plans can become apart of the services which are provided within the community by social workers who are employed by the state. They can partner with case managers and their individuals to derive long-term care plans, and this takes some of the strain off of case managers. Advocacy groups such as non-profit organization can bring forth the issues of long-term care to local and state level governments, so programs and initiative can be set forth. This is important for people to start looking at long-term care early, and avoid future cost which can become a burden on their loved ones or an unmet need for care.
This paper will answer questions related to social problems such as what conditions or needs does the agency address and what measurements or criteria are used to document the problems or issues. In addition, the information for a nursing home can be overwhelming and all the aspects that need to be known, however, I did the best I could for answering all the questions and it can get confusing when reading all the sources of information. Furthermore, other questions to be addressed include is how the community could be harmed if the conditions got worse, and questions related to evidenced-based practices, and any research that I might recommend to be undertaken to get more knowledge about social problems at the agency.
I engaged in providing service to older adults at “The Place at Pooler”. This is a self-owned, assisted living facility, which offers long-term stays and short– term rehabilitation care. They have three different locations, The Place at Deans Bridge, The Place at Martinez, and The Place at Pooler. The facility I went to in Pooler has a hundred twenty–two beds. They have private rooms as well as semi-private rooms available. The facility has shower rooms located on each the patient’s hall that are equipped with showers and whirlpool tubes for their residents. They also try to make the resident fill at home, so in their facility they have a media room, dining room. Their residents also have access to the beauty and barber service, and housekeeping and laundry service. The activities planners at “The Place at Pooler,” take their residents on field trips, and have recreational activities for them. Their residents also have access to the beauty
All they want is to take part of society as a normal individual. The disability-movement has fought for the disabilities rights throughout the years and has achieved goals such as accommodation of architectural infrastructures to serve better people who are physical impairments. The public policies have been great accomplishments because it has helped people with disabilities to be part of society. The disability-movement points out the healthcare finance policies have taken freedom away from the disability community, “ Health-care financing policies force disabled people into Institutions and nursing homes rather than funding independent living. Income-maintenance and public health-insurance policies include “disincentives” that penalize disabled individuals for trying to work productively.”(p.4). The government has done a great job on protecting disabled individuals’ rights. However, the health-care system has isolated this group even more by restricting the level of productivity that they have within the system, as a result this medical model marginalizes this group of people and this program available for this community does not fully address their issues.
As Scotland’s approach to the demographic challenges of an increasing ageing population, they set out a programme called Reshaping Care for Older People: a programme for change 2011−2021 (Scottish Government 2011). This programme goal is to enhance the well-being and independence of older people in a home setting.
